Update: ok I see what happened. I mounted the voltage input jack (wired center negative) on to a metal plate along with the RCA jacks. The plate is shorting the voltage input, that's why I didn't get any readings. Now the solution would be to get a plastic input jack so it's isolated from the panel ...

Update: It appears it was the voltage regulator causing no voltage readings. I replaced the fuse again after replacing the regulator and it worked, but for good measure I swapped out all 6 caps on my revision and the thing is running great. Nothing getting hot so far. A short in the 5v rail would show as continuity with GND? Because I tested to see if GND and 5v were shorted and they aren't. :(
