I know I'm bumping an old thread, but still.
I don't think Batman for NES is that far from the movie. It has some major differences, but there are still a lot of elements from the movie. The stages mostly follow locations from the movie. The first stage are the streets of Gotham, the second level is the Axis Chemical Factory. The last stage is the bell tower. The intro and the last cutscene follow the movie, although the ending in particular takes some liberties.
I think the game, that follows the movie the most has to be the arcade game by Atari. The locations and the enemies are based on the movie, the soundtrack consists of music from the movie and there are even digitized voice samples from... well... the movie.
